How to start care for
your Family
A Referral is Submitted on Your Behalf
Your care coordinator, social worker, or health plan submits a referral to 24 Hour Home Care on the Medi-Cal members behalf.
We Confirm Your Loved One's Eligibility
Our team works directly with the member’s family and the health plan to verify eligibility and get authorization.

FAQs
How do we select a caregiver?
Through our Agency with Choice model, families can choose someone they already know and trust — a family member, a friend, a neighbor — and we employ that person as a paid caregiver.
Is there a cost to Medi-Cal members or their families?
No. There is no cost to eligible Medi-Cal members or their families. Services are funded through CalAIM Community Supports as part of the member’s Medi-Cal managed care benefit. We work directly with the health plan to secure authorization, manage billing, and onboard and pay the family’s chosen caregiver.
What does a caregiver actually help with?
Caregivers assist with the everyday tasks that help your loved one stay safe and comfortable at home — bathing, dressing, meal preparation, medication reminders, light housekeeping, and companionship. Services are non-medical and tailored to what your loved one needs. See a full list of services.
What if we don't have someone in mind to be the caregiver?
You don’t need to have someone lined up before you reach out. Caregivers can be a family member, neighbor, community member or friend. Think about the people in your loved ones life who may help them with visits, meals, or taking out the trash. We’ll work closely with you to identify a trusted caregiver.
What if my loved is already enrolled with IHSS?
Community Supports and IHSS are designed to work alongside each other, not replace one another. If your loved one already has IHSS, we can fill the gaps with additional hours if maxed out with IHSS, supporting while waiting an IHSS reassessment, or respite care for you as the primary caregiver. Your existing IHSS hours and benefits stay completely intact.
