Families seeking
Servicios para Discapacitados
Families know their loved ones best. Our program allows them to choose someone familiar to be their caregiver while we handle onboarding, training, and ongoing support.
Family-selected care: Faster starts, fewer turnovers, better outcomes
We partner with families and all 21 Regional Centers across California to provide care that respects each individual’s needs, culture, and preferences.
With fast response times, multilingual support, and local expertise, we make care easier and more accessible for families across California.
Disability Services that help individuals thrive
Our person-centered services are designed to support independence, encourage meaningful community involvement, and promote caregiver wellbeing. Together, these elements create a stronger, more balanced approach to care.
Getting started is simple
Find Your Regional Center
Contact your local Regional Center by following the link below. The Regional Center will assess your eligibility and if deemed eligible for services, a Service Coordinator will determine the services and hours available to your family.
Receive Referral and Authorization
The Service Coordinator then sends the referral to 24 Hour Home Care and our care team reaches out to you and your caregiver.
Onboard Caregiver and Start Services
Families select a caregiver they know and trust. This can be a family member, friend, or community member, and more than one caregiver may be selected. Once onboarding is complete, caregivers can begin services, log hours, and receive payment.
