Everything Changes When Veterans Choose Their Own Caregiver

Proudly serving Veterans through the Community Care Network since 2011, helping them stay safe and independent at home with a caregiver they trust.

The Best Home Care Starts with Trust

Veterans deserve care from someone who understands their values, service, and story. We give them the power to choose their caregiver, leading to more consistent and lasting care.

How We’re Different

Care Powered by Choice
We empower Veterans to choose their own caregiver, like a fellow veteran, family member, or neighbor.

True Veteran Advocacy
We work as an extension of care teams, coordinating services and ensuring Veterans receive the right support at every step.

Faster, More Sustainable Care
When care starts with someone familiar and trusted, services begin faster and care remains continuous.

Frequently asked questions

Who qualifies for VA Homemaker & Home Health Aide program?

Eligibility is primarily based on functional needs and medical status, not income. To qualify, Veterans must:

  • Be enrolled in VA healthcare; enrollment in VA medical benefits is a prerequisite for H/HHA services
  • Veterans must have difficulty performing at least one daily task (activity of daily living) such as bathing, dressing, toileting, meal prep, light housework, medication reminders)
  • Be assessed by a VA healthcare provider
  • A physician, nurse, or social worker will determine whether in-home support is medically necessary
  • Live in a setting where in-home services are feasible
  • This program is designed for veterans living in the community, not in nursing homes

Approved services are covered and funded by the VA, so there is no out-of-pocket cost for eligible and approved hours.

24 Hour Home Care’s HHA program provides non-medical in-home support, including:

  • Bathing and dressing assistance
  • Mobility support
  • Meal preparation
  • Light housekeeping
  • Medication reminders
  • Companionship and supervision

Yes, Veterans can select their own caregiver. Often a family member, friend, neighbor or fellow Veteran who may already be supporting the Veteran. The program allows Veteran-selected caregivers to be compensated for approved hours provided they meet minimum requirements.

Our team will work with you to identify someone in your existing network who may be a good fit, such as a family member, friend, neighbor, or fellow Veteran. If you don’t have someone in mind, we may also be able to connect you with a caregiver from our network, depending on availability in your area.

We support Veterans throughout California and continue to expand our VA footprint across the state.

Yes. Because Veterans can often choose someone they already know, we’re able to support many rural and traditionally underserved communities where caregiver availability can be limited
