Complete the referral form. It takes a few minutes and can be done from anywhere. You’ll receive confirmation right away.
Member Outreach and Authorization
Our team works directly with the Medi-Cal member’s family and the health plan to verify eligibility, get authorization, and select a caregiver of their choice.
Care starts with someone trusted and familiar
Once everything is in place, the caregiver the Medi-Cal member has chosen is onboarded and care begins. Familiar, consistent, and tailored to their needs.

What information do I need to submit a referral?
You’ll need the member’s name, date of birth, Medi-Cal ID, health plan, IHSS status and a brief description of their care needs. The more context you can share upfront, including clinical documentation, supporting the members needs, the faster we can confirm eligibility and get care started. Submit a referral
How quickly will you follow up after I refer?
Our team reaches out to the member’s family within one business day of receiving the referral.
Can I refer a member who is still in a facility based setting?
Yes! The earlier the better. Submitting a referral before discharge gives our team time to confirm eligibility, secure authorization, and have a their caregiver ready when your member gets home.
Which health plans do you work with?
We are contracted with all 24 Medi-Cal managed care plans across all 58 California counties. We work directly with the member’s plan to secure authorization so your team doesn’t need to navigate that process.
What if the member doesn't have a caregiver in mind?
Submit the referral. We will work with the member to identify a familiar caregiver. Caregivers can be a family member, neighbor, community member or friend.
The member has already applied for IHSS. Should I still submit a referral?
Yes! The sooner you refer, the better. IHSS can take weeks or months to be approved. We can step in and fill that gap so care starts sooner.
