This whole-person approach recognizes something social workers and case managers have always known; that health doesn&#8217;t happen in a doctor&#8217;s office alone. This is the initiative most directly relevant to elderly clients needing in-home support.<\/li>\n\n\n\n<li><strong>Behavioral Health<\/strong> expands access to mental health and substance use disorder services, integrating them more seamlessly into a member&#8217;s overall care.<\/li>\n\n\n\n<li><strong>Justice-Involved Reentry Initiative<\/strong> and <strong>Long-Term Care \/ MLTSS<\/strong> serve members transitioning from incarceration or navigating complex long-term care needs across Medicare and Medi-Cal.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Together, these initiatives form a connected system where a member&#8217;s Medi-Cal care, mental health, daily living needs, and social circumstances are addressed as a whole rather than in isolation. The strength of CalAIM is that each layer supports the others, creating a more complete picture of care for members who have historically fallen through the gaps.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For case managers and social workers working with senior, Community Supports is the CalAIM initiative most directly tied to keeping people out of facilities and in the place where they recover faster, stay more connected, and maintain their independence: home. Care that starts familiar, with someone a member already knows and trusts, is more stable, more consistent, and less likely to end in a preventable hospitalization or facility placement. It is worth understanding in detail.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What is CalAIM Community Supports?<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Through CalAIM, Medi-Cal managed care plans can offer members up to 15 Community Supports, including non-medical services designed to address health-related social needs, support daily living, and help members avoid higher, costlier levels of care. These two services directly tie to keeping members safely at home and out of a facility or hospital.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><em>DHCS data shows that Respite Care members experience 43.4% lower inpatient costs, and Personal Care and Homemaker members show 28.9% lower rates of avoidable institutionalization. By addressing these needs consistently and early, these services reduce the gaps in care that too often lead to preventable hospitalizations and facility placements. All services are authorized by the member&#8217;s health plan at no cost to the member. Services include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Bathing, toileting, and grooming<\/li>\n\n\n\n<li>Meal preparation<\/li>\n\n\n\n<li>Medication reminders<\/li>\n\n\n\n<li>Supervision and socialization<\/li>\n\n\n\n<li>Light housekeeping<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Atenci\u00f3n de respiro<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Every caregiver deserves a break. Medi-Cal funds specialized hours to provide short-term in-home caregiver relief to avoid a member&#8217;s placement in a facility.
